We require a medical form for each Kamper to be completed prior to arrival so that our Health Services Team can treat any needs Kampers may have when visiting a Kanakuk Health Center.
To complete your Kamper’s Medical Form, login to your MyKanakuk account online or on The Kanakuk App, navigate to your Kamper’s dashboard, and select the “Medical Information” tab. If this information hasn’t been completed, an alert will show on your account, as this information is required for your Kamper to check-in at Kanakuk. Please note — the only immunization date needed on the form is your Kamper’s most recent tetanus shot.
